Emergency Permit Form
What is the Emergency Permit Form?
There is an emergency provision in the 310 law to handle actions necessary to safeguard life or property, including growing crops, during periods of emergency. If a person takes an emergency action, the conservation district must be notified within 15 days in writing of the action taken and why.
Conservation district review:
The emergency action will be reviewed and inspected by the conservation district. The district will decide whether the action was appropriate, must be modified, or must be removed and/or replaced.
Limitations of Emergency Provisions:
The emergency provisions of the 310 Law do not apply to other permits, authorizations, or certifications that may be required for this activity. This notification does not imply landowner permission.
Applicant Responsibilities:
The applicant is responsible for acquiring all other authorizations necessary to carry out this work and for obtaining landowner permission.

4. Enter a password. Passwords must contain at least 8 characters, an uppercase and lowercase letter, and a special character.

Excellent! You have created your account. Now you need to verify your email address before accessing the platform.
7. Go to your email inbox, open the received email from Gilly and click on 'Verify account'.
8. Click on 'Log in'

Dashboard
1. 'In Progress' tab:
Here you can see all of the permits that you are currently editing. When you want to continue filling out your application, click on 'Resume'
2. 'All permits' tab:
Here you can see all of the permits you have ever created. You can check your permit applications to see how far along they are in the review process.
Apply for a permit
1. Click on 'Apply for a permit'
2. On the first step ("CD"), select your local Conservation District from the list.
3. Navigate the form using the 'Next step' and 'Previous step' buttons at the top.
4. On the third step ("Project location"), you have to pinpoint the location where the project will take place. To get an accurate location, search the address or name of reference in the map's search field.
5. You can also move the pin through the map by drag & drop or with a click on the map. The location parameters will be automatically filled.
6. Make sure you click on 'Save progress' so that your information is saved. If you do not have all the required information readily available, you can save your progress and come back to finish it later.
7. Once you have filled the entire form and uploaded the required files, you can proceed to sign the permit application
8. If there are missing required fields, you will not be able to proceed to signing the document.
The red tabs will highlight the sections that have missing information. Required fields are marked with an asterisk (*). These are required by the DNRC.
Electronic signature
1. After filling the application form and making sure all of the required fields are completed, click on 'Sign permit'
2. Read through the preview of the application form. Once you are certain it is okay for submitting, click on 'Continue'.
3. Click on 'Sign digitally'
4. Choose the signature method (Type, Draw or Upload) and complete it.
5. Click on 'Sign document'
6. Click on 'Sign and submit'
Congratulations! Your permit application has been submitted.
Manual signature
1. After filling the application form and making sure all of the required fields are completed, click on 'Sign permit'
2. Read through the preview of the application form. Once you are certain it is okay for submitting, click on 'Continue'.
3. Click on 'manual signature'
4. Click on 'Okay'. The file will be downloaded to your computer.
5. Once you review, print and sign the permit physically, proceed to scan it and upload it to Gilly.
6. Once you have uploaded the signed document, click on 'Continue'.
7. Click on 'Sign and submit'
Congratulations! Your permit application has been submitted.

Dashboard - Pending tab
In this tab you can see all of the permits that are in the first step of the review. Some applications haven't been read yet (status is "Under review") or have been given a quick look and determined that are not ready to move to the next step unless some changes are made (status is "Changes requested").
What can I do in this dashboard?
Review applications: by clicking on a permit application, you will see the filled form to begin the assessment.
Change application status: in this case, you can change the applications that are 'Under review' to:
'Changes requested' if the application form is missing a key part of the information or requires important changes. You can contact the applicant via email to explain what changes need to be made.
'Pending inspection' if the application seems okay after a first assessment and needs an on-site investigation.
'Rejected' if the application is not feasible at all.
Search: you can search permits by the application number, applicant name and landowner name.
Sort: you can sort the table by any column by clicking on the arrow next to the column title.
Filter: you can filter the permit list by status, stream and submission date. Select multiple filters to see any specific information you might need. To delete the filters just click on the 'X' next to the filter button.
Dashboard - Reviewed tab
In this tab you can see all of the permits that are in the second step of the review. Some applications are waiting for the on-site inspection to take place (status is "Pending inspection"), while others have passed the inspection and are waiting for the CD Board determination (status is "Ready for CD").
What can I do in this dashboard?
Review applications: by clicking on a permit application, you will see the filled form and be able to access the inspection files to complete the on-site report.
Change application status:
You can change the applications that are 'Pending inspection' to:
'Changes requested' if the application form is missing a key part of the information or requires important changes. You can contact the applicant via email to explain what changes need to be made.
And the ones that are 'Ready for CD' to:
'Approved' if the project has been approved to start.
'Approved with modifications' if the project has been approved to start but needs minor modifications.
'Denied' if the application is not feasible at all.

In this tab you can see all of the permits that have completed the review process. You will find some applications were approved, some were approved with modifications and some were rejected.
What can I do in this dashboard?

Search: you can search permits by the application number, applicant name and landowner name.
Sort: you can sort the table by any column by clicking on the arrow next to the column title.

Dashboard - Historical data tab
In this tab you can see all of the permits that were digitalized by the Conservation District. These are permits that have completed the review process out of Gilly.
What can I do in this dashboard?
Review past applications: by clicking on a permit application, you will see the filled form and will be able to edit it.
Search: you can search permits by the application number, applicant name and landowner name.
Sort: you can sort the table by any column by clicking on the arrow next to the column title.
Filter: you can filter the permit list by status, stream and submission date. Select multiple filters to see any specific information you might need. To delete the filters just click on the 'X' next to the filter button.
CD/Agency information
In the header of any permit application, you will find the 'CD/Agency use' button.
When entering that section, you will be able to complete specific information that is mainly relevant for the CD. The applicant does not have access to this information.
Make sure you save your progress after editing this form and it will be visible throughout the entire review process.
Inspection Report
In the header of permit application with the status of 'Pending inspection', you will find the 'Inspection files' button.
When entering that section, you will be able to complete the on-site inspection, relevant for the CD board to make a determination.
The first question is whether the project actually needs an inspection. If it is a recurrent project and there is no need to make an on-site report, you can select 'No' and click on the FINISH button. The project status will automatically change to 'Ready for CD' to continue down the review process.
All of the applicant and project location information is available in this section for easy access.
Fill out the on-site inspection, review considerations and recommendations.
Upload relevant attachments, such as pictures of the project location.
Go to the next step to sign the inspection. The name and signatures of the inspector, the applicant representative and the Fish, Wildlife and Parks representative are required.
Electronic signature:
Write down the name and click on 'Sign inspection'. The signature screen has three different methods of signing the document: by typing your name, by drawing your signature or by uploading a file with your signature.
Choose your preferred method and click on 'Sign document'. Repeat the process for the three signers and click on the FINISH button. The project status will automatically change to 'Ready for CD' to continue down the review process.
Manual signature:
Click on the 'manual signature' button on the information block at the top of the page. The completed inspection form will be downloaded to your device.
Once you print, sign and scan the document, click on the menu at the top of the page and select 'Upload'.
Browse your computer and upload the signed inspection form. Click on 'Continue' to officially submit the report.
The project status will automatically change to 'Ready for CD' to continue down the review process.
Inspection's section
Are you a CD inspector? Now you can access Gilly from your phone and take it with you to perform the on-site inspections. Don't worry about internet signal, the system works offline and uploads all of your progress to Gilly once you get back online.
1. Go to the 'Inspection' tab on the header.
2. Scan the QR code with your phone.
3. Log in to your account.
4. Add a shortcut on your phone to get a faster access next time! Click on 'share' and find the option 'Add to Home Screen'.
5. You will find a list of permit applications filtered by the status of 'Pending inspection' for your district.
To perform the inspection just click on the permit, fill the inspection form with the review considerations and recommendations, upload relevant attachments, and sign it. The name and signatures of the inspector, the applicant representative and the Fish, Wildlife and Parks representative are required. You can use any of the three digital signature methods.
6. Once everyone's done, click on 'FINISH'
7. The 'Inspection status' on the list will automatically change to 'Completed', and you will find the application as 'Ready for CD' in the Gilly web platform.
8. If you're offline and have fully completed one or more forms, once you recover signal the system will automatically sync with the web platform so everyone on your team can find the updated inspections.
Add historical data
In the header of the dashboard you can find the 'Add historical data' button.
The goal of this section is to add completed application forms that are already approved to get them digitalized.
To submit a new historical form, follow these steps:
Click on the 'Add historical data' button.
Complete the information requested on the form. The required fields are signaled with an asterisk.
Once you are finished, click on the 'SUBMIT' button.
Congratulations! You can now find the submitted historical form on the 'Historical Data' tab in the dashboard.
Help applicant
When an applicant is having trouble submitting a permit application and contacts the CD, you can offer to help them by impersonating them in the Gilly platform. This means that you can get on their account and create a permit application or continue filling an existing one.
To do this, follow these steps:
1. Click on the 'Help applicant' button found in the header of the dashboard.
2. Enter the applicant's email and click on 'Continue'.
3. You will see every application with the status 'In progress' or 'Changes requested' from the applicant which corresponds to your CD. You can create a new application from scratch by clicking on the 'New permit' button, you can edit an existing application by clicking on the 'Edit' button or you can create a new application by duplicating a previously existing one by clicking on the 'duplicate' icon next to the edit button.
4. Help the applicant complete the permit application and have them sign the form using their own account.
Maps
In the 'Maps' section on the header, you can find a map locating every permit application. The different colors indicate the review status.
What can I do in this map?
Change the base map: to suit your preferences, you can change the base map to satellite version by clicking on the map miniature in the bottom left-hand corner.
Go to my location: by clicking on the first button in the bottom right-hand corner, the map will center in your location. Make sure you allow the browser to access your location!
Zoom in & out: bellow that button you will find + and - buttons to zoom the map in an out.
View permit information: by hovering over the pins on the map you will see a summary of the permit application. If you click on the pin, a panel with more extensive information on the permit will be shown. Click on the 'View permit' button on the panel to see the full permit application.
Add layers: by clicking on the layer button in the top left-hand corner, you will find a list of predetermined layers. Click on the eye button to activate each one and see the information overlap on the map.
Add filters: by clicking on the filter button in the top left-hand corner, you will find a list of parameters to filter by. Select your preferred options and click on 'Apply filters'. You will see that the remaining pins on the map are the ones that match the combination of filters selected. To clear the filters just click on the 'X' button on the filter you want to erase at the top of the map.
Polygon tool: if you want to see the information on a group of permits at once, you can use the polygon tool. By clicking on the polygon button in the top right-hand corner, you will activate the tool. Click on the map to make the selection you desire and do a double click when you finish. A list of the grouped permits will be shown in a lateral panel.
